You can still purchase a brick to support the Hazen Park playground. We will be collecting forms until we have enough for another order. Please fill out the attached form and return it with the appropriate payment to:

You can also drop it by the office at 127 W. Washington St. Regular office hours are 8:00am – 11:00am; Monday – Friday. If the office is closed, you can slide the paperwork under the door. Send an email to admin@villageofphilo.com that you dropped it off.
